NOTE: Please pay close attention to the instructions below, as it is required by HIPAA to correctly fill out the appropriate form to authorize the disclosing of health records.

If your insurance is NOT provided through the University of Alabama, OR you wish for the UA Student Health Center to disclose health records to another party, please download and fill out the following form as needed.

If your insurance provider is through the University of Alabama (United Healthcare Student Resources) and you wish to disclose health records to the Student Health Center, please download and fill out the following form. NOTE: The information already provided in the form MUST NOT be adjusted in order to stay in compliance with HIPAA regulations.

Free Flu shots today from 12-4:30 at Paty Hall, Wednesday we will be at Presidential and Thursday Riverside Residential Halls. All clinics from 12-4:30 pm. Free flu shots are also available at the SHC while supplies last.

The flu season has begun and the cases are generally mild, lasting about three days. You may have the flu if: you have a fever of 101 or greater, you have a sudden onset of headaches, muscle aches, and are extremely tired, and a cough is becoming worse. If this is true for you, please call the Flu Hotline at 205-348-6202 to talk to a nurse.. If you have been treated and are getting worse please come to the Student Health Center as soon as possible.

The SHC has added more services including a female OB/GYN doctor. Her name is Kristy Graettinger. Visit the Medfusion link to complete registration, medical history forms and to schedule an appointment.

Women’s Health Services include contraception, pelvic exams, pap smears, and counseling. Other Student Health Center services

include urgent care, ADHD clinic, allergy, alcohol & drug education, health & wellness, eating disorders, dermatology, nutrition therapy, peer health education, pharmacy, physicals, psychiatry, X-ray and Laboratory, STD testing, and a sports injury center.
